Book An Assessment
If you would like to book your assessment, please fill in and submit the downloadable booking form below and then email it to us at matrix@emqc.co.uk.
When completing the booking form please ensure the designated contact (name) you put is the key contact who will oversee your organisation's Accreditation against The matrix Standard. This is usually a senior member of staff who your matrix Assessor can contact to plan your assessment in detail.
In addition to information about your organisation, you will also need to have to hand the total number of people involved in the management and delivery of your organisation and its information, advice and guidance service or services. This should also take into account any senior managers, board members that are involved in planning your organisation's overall aims and objectives, and the total number of people delivering your service. This should include all part-time, full-time staff and volunteers
Once you complete the downloadable form below and submit it to matrix@emqc.co.uk, you will then be committed to an assessment against the matrix Standard and as such you are subject to emqc Ltd's standard Terms and Conditions and Cancellation Policy.
